PPHC is excited to partner with HealthInfoNet in an effort to create better, easier, and safer healthcare solutions for our patients.

What is HealthInfoNet?

  • HealthInfoNet manages a secure computer system used by the healthcare community to share health information with the purpose of providing you with better and safer care.
  • PPHC is only RECEIVING information from other facilities (such as a referral, ER visit, or MD visit outside PPHC). PPHC is NOT SHARING any of your health information with other facilities.

 

What are the benefits?

  • HealthInfoNet helps your healthcare providers work together to make informed decisions about your care, especially in emergencies and transitions of care.

What is in my record?

  • Your health record includes information about your prescription medicines, allergies, test results, visits and documents, procedures, and more.
  • It also includes demographic information, including your name, date of birth, address, and phone number.
  • Sensitive information is only included if you say it’s okay, or in the case of an emergency.

 

Is it private and secure?

  • HealthInfoNet takes every action to keep your records private and secure.
  • Information is encrypted and sent over private and secure computer connections.
  • Information that identifies you will not be sold or added to any mailing lists.
  • Only those involved in your health care are authorized to access your record.

 

What if I don’t want a HealthInfoNet record?

  • You retain control over your personal health information.
  • If you do not want your health information in a HealthInfoNet record, you may choose to opt out. Simply fill out an “opt-out form,” found online at hinfonet.org/for-patients/#know, or call HealthInfoNet’s Customer Support team at 866-592-4352.
  • Please note that if you opt-out, your health information will not be available to participating providers, even in the case of an emergency.
